Disclosure Statement, Form CASB-DS-1, is not included in this solicitation package. Any offeror meeting the criteria for concurrent submission of the Disclosure Statement may download a copy of the form from the Office of Management and Budget (http://www.whitehouse.gov/omb/procurement/casb_ds-1.pdf). Offeror is responsible for reproducing the completed form in sufficient number of copies required for submission. No extension of the closing date of the solicitation will be granted on account of the requirement for submission of the Disclosure Statement.
